2009 Polar Plunge Release Form

This is the registration form for the 2009 Polar Plunge to be held at the Boardwalk in Carolina Beach, NC. on Feb. 21, 2009 starting at 11:00 am.

You must be over 18 to plunge without a parents or guardians signed release. See link below for the printable form. Please have a parent or guardian sign the release form and bring it to the registration desk at the Plunge.
